You will need to provide the following details as well as a photo of yourself not smaller than 1" x 1" and a written signature sample. (click for info on photo requirements)If you do not send a signature sample, we will make up your signature for you. (Any ID card details you do not know, leave blank and we will provide these details for you.)
Details marked in red are required to get your order filled.
Details marked in yellow are required by some states, but not all.
Details marked in green are required by all states, but are commonly left blank & filled in by RealFakeID.com to ensure proper license formatting.

You may print off this Order Form or write the above details on paper. For multiple orders, simply print off one form for each ID.
Payment
We accept cash or money orders in US currency. Western Union Money Orders are preferred payment. Personal cheques can not be cashed.If you are sending a money order, be sure to leave the "Pay to the order of" or "Payee" line empty.
Regarding Money Orders: We will not accept US Postal Money Orders, unless they say International at the top, and are pink in color. This is due to the fact that normal US Postal Money Orders (green at the top) are not valid outside the USA.
Pricing
Each ID is $90 USD. Shipping via Airmail is included for free. ExpressPost 2-5 day trackable delivery is also available for a $16.50 USD flat fee per order. The listed price is the price you pay. All prices are in USD, and at no additional cost we will mail to any country.
